University Forms

All campus forms can be found online through the Registrar's office website. The forms that require advsior assistance are: 

  • Change of Major (
  • Drop/Withdrawal Process 
  • Excess Unit Form 
  • Petition for Retroactive Withdrawal
  • Time Conflict

BASE Office Forms 
  • Option Contract: The option contract form is used to declare your option once students have completed their Pre-Business courses. 
    Note: All students must complete all Pre-Business courses and have a 2.25 Campus and Cumulative GPA to submit the option contract

 

  • Course Substitution Form: Please contact your assigned advisor to discuss courses taken off campus to the Craig School of Business departments for review.

 

  • Thrid Attempt Form: Please use the following link to request a Third Attempt Form to be submitted on your behalf. Please do one form per course. You will be emailed once the submission has been sent on your behalf. If an advisor sees any issue with your form, you will be contacted via email.
    • DS 123 Third Attempt form: For students needing to take DS 123 for a third time, please complete the following form in addition to the Thrid Attempt form. 

 

 

 

 

 

Planning Forms

 

  • Course Mapping Sheet: This form can be used with the Degree Progress Report and the Academic Flowchart to plan out the remaining terms at Fresno State.
