The forms listed below are the most frequently used forms by the students in the Craig School of Business. 

Community College Articulation Sheet
Intended for students who are looking to take their Pre-Business or Fashion Merchandising courses outside of Fresno State at a local California Community College. If you are unable to find the California Community College you are planning on attending, please use www.assist.org to find the equivalent course. 


Course Mapping Sheet
This form can be used with MyDegreePlan and the Academic Flowchart to plan out the remaining terms at Fresno State. 

Change of Major/Minor Request Form 
This form is used to change your major into or add a minor from the Craig School of Business. Students must meet with an advisor during walk-in hours or make an appointment to have form signed off.
*Note* form must be printed double sided to be accepted by the Office of the Registrar. 

Drop / Withdrawal Form
This form is used to drop a course after the drop online deadline has passed or to withdraw from a course prior to the withdrawal deadline. Registration dates and deadline information.

Excess Unit Form
This form is used to enroll in additional units after open enrollment if the student does not meet the unit increase granted by the student's GPA. All Craig School of Business students must meet with an advisor to have their form signed. 

Third Attempt Approval Form
This form is used to enroll in a course for the third time at Fresno State. All Craig School of Business students can complete the form over the counter and do not need to make an appointment for this form.

Option Contract

This form is used to declare your option once students have completed their Pre-Business courses. 
Note: All students must complete all Pre-Business courses and have a 2.25 Campus and Cumulative GPA to submit the option contract. 

Course Substitution Form
This form is used to submit courses taken off campus to the Craig School of Business departments for review.
